The application collects your name, work email, role, organization, state or region, and any optional program information or comments you provide. Please do not include individual athlete records, health information, or other sensitive personal information.

Henzer uses application information to review partner interest, understand organizational nutrition needs, and contact you about Formula 1.0 and the Founding Performance Partners program. Applying does not imply a partnership or endorsement.

Form hosting and processing

This website and its application form are hosted and processed by Netlify. Submitted information is made available to the Henzer site administrator through Netlify Forms. Netlify may process technical request information to provide hosting, security, and spam protection.

Your choices

Optional fields may be left blank. You may request that Henzer stop contacting you or delete your application by replying to a program message from Henzer. For an initial inquiry, use the application form with only the required information and describe your request in the comments.

Retention and access

Application information should be accessible only to authorized program administrators and retained only for as long as needed to evaluate and manage the partner program. No advertising analytics or tracking cookies are added by this website.
